The "E-referral" tab is used to issue electronic referrals for the patient. The tab shows a list of referrals issued during a given visit and two forms for issuing referrals:

  • referral for consultation
  • referral for examination or treatment
  • referral for rehabilitation
  • referral for spa treatment

By default, the "E-referral" tab displays dark bars with the names of the available referral types. To display the form for a given type of referral, click on the appropriate bar and the appropriate form to be completed will unfold. In this form:

  • enter the title, that is, the headline that should appear on the referral; when entering a title, the system displays a list of hints from previously used titles and the correct title can be selected from the list of hints
  • except for a referral for spa treatment, the specialty of the cell to which the referral is addressed should be specified; when entering the name of a specialty, the system displays a list of hints and the appropriate specialty should be selected from the list of hints
  • the primary recognition code and description must be entered; by entering a fragment of the name in the field for the recognition code, the system displays a list of hints, after selecting the recognition from the list of hints, the code and description are automatically filled
  • optionally, the code and description of the secondary recognition can be entered in the same way as for the primary recognition
  • it is possible to indicate the patient's guardians, whose data are to be attached to the referral (provided that the guardians' data have been entered in the patient's card)
  • if the referral is to be carried out urgently, check the "Urgent" bo
  • if the referral is to be accompanied by the patient's insurance data, select the "Add patient insurance data" box; in order for the referral to be used to arrange a visit financed by the National Health Fund, it must contain the patient's insurance data (otherwise it can be used only for a commercial visit); during a commercial visit, it is not possible to attach the patient's insurance data to the referrals, except for referrals to a hospital, where such a possibility exists
  • in the case of a referral for examination or treatment, it is possible to indicate the procedures to be referred to; to indicate the procedures, in the "--- procedure ---" field, enter a fragment of the procedure name and then the system will display a list of hints - select the appropriate procedure from the list of hints, additionally, you can indicate the area of the body and the side of the body to be performed by the procedure, then click the "Add" button - by repeating the above steps many times, you can add any number of procedures to the referral (the list of added procedures is displayed above the "--- procedure ---" field and a red icon with the "×" sign is visible next to each of the procedures allowing to remove a given procedure from the referral in the event of its incorrect introduction)
  • in the case of a referral for spa treatment, the results of diagnostic / imaging examinations should be entered, indicating the completed examination from the collection point / attachment from the "History" tab in the patient's card
  • in the case of a referral for spa treatment, information about vaccinations should be entered - to do this, click the "Add vaccination" button and then the field for entering the ICD-9 procedure code corresponding to the vaccination performed will be displayed - by entering a fragment of its name in this field, the system displays a list of hints from which to choose the correct position; to add another vaccination, use the "Add vaccination" button again

There are three buttons below the form:

  • the button with the floppy disk icon allows you to generate and save a referral to the database
  • a button with a paper airplane icon allows you to generate and save a referral to the database and send it to the P1 platform
  • a button with a cross icon allows you to resign from issuing a referral (clicking it will delete the data entered into the form)

After clicking one of the first two buttons, the system performs the required action and the generated referral is displayed on the list of referrals above the form. For each referral, the following icons may be visible on the list of issued referrals:

  • send - the icon allows you to send to the P1 platform a referral previously generated and saved to the database (when you click it, the referral is sent to the P1 platform and the set of icons visible next to a given referral is updated); the icon is visible only for referrals that have not been sent to the P1 platform
  • edit - the icon allows you to edit the referral data (when you click it, the referral data form is initiated with the referral data, which can be modified and saved); the icon is visible only for referrals that have not been sent to the P1 platform
  • delete - the icon allows you to remove a referral (when you click it and confirm your intention to remove it, the referral is removed from the list); the icon is visible only for referrals that have not been sent to the P1 platform
  • print (black) - the icon allows you to print the referral (when you click it, the referral view opens in a new window, enabling to print it); the icon is visible only for referrals that have been sent to the P1 platform and have not been canceled on the P1 platform
  • print (blue) - the icon allows you to print the informations about referral (when you click it, the informations about referral view opens in a new window, enabling to print it); the icon is visible only for referrals that have been sent to the P1 platform and have not been canceled on the P1 platform
  • cancel - the icon allows you to cancel the referral on the P1 platform (when you click it and enter cancellation reason, the referral is canceled on the P1 platform and the set of icons visible next to the referral is updated); the icon is visible only for referrals that have been sent to the P1 platform and have not been canceled on P1 platform
  • correct - the icon allows you to issue a referral correction (when you click it, the referral data form is initiated with the referral data, which can be modified and saved); the icon is visible only for referrals that have been sent to the P1 platform and have not been canceled
  • print (red) - the icon allows you to print the referral cancellation document (when you click it, referral cancellation document view opens in a new window, enabling to print it); the icon is visible only for referrals that were sent to the P1 platform and then canceled on the P1 platform

Information about a given icon is displayed in a tooltip when you hover the mouse cursor over the icon. The following information icons may additionally be displayed on the list for the type of referral:

  • warning - the icon informs about the alerts regarding the referral returned by the P1 platform when sending / canceling the referral; the icon is displayed next to the type of referral; the content of the warning is displayed in a tooltip when you hover the mouse over this icon
  • correction - the icon informs that a correction has been issued for a given referral

For each referral that has been sent to platform P1, the referral code issued by that platform is displayed. By default, its first four digits are visible, with two arrows enabling the display of the full code (its hidden part is the patient's PESEL number). In addition, for each referral that has been sent to the P1 platform, icons are displayed informing about whether the patient uses the Patient Online Account and whether he receives notifications from the P1 platform:

  • IKP icon (green) - the patient uses the Patient's Internet Account
  • IKP icon (red) - the patient does not use the Patient's Internet Account
  • envelope icon (green) - the patient receives notifications from the P1 platform via email
  • envelope icon (red) - the patient does not receive notifications from the P1 platform via email
  • balloon icon (green) - the patient receives notifications from the P1 platform via SMS
  • balloon icon (red) - the patient does not receive notifications from the P1 platform via SMS

Information about a given icon is displayed in the tooltip displayed after hovering over it with the mouse cursor.

The "Back" and "Next" buttons visible at the bottom of the tab allow you to switch accordingly to previous and next tabs in the visit card.
