Patient - personal and identification data

 

In the "patient" tab there is a form with personal and identification data of the patient. Above the form, there are information icons. The number of icons depends on the patient data. The scope of the icons is following:

  • juvenile patient – by this icon marked are patients up to 18 years of age and patients after 18 years of age having special permission to use the clinic for children
  • notorious not setting yourself up for a visit - by this icon marked are patients who failed to appear for at least 3 consecutive appointments
  • entered declaration to family doctor – by this icon marked are patients having entered (still not send to the NFZ) a declaration to family doctor
  • entered declaration to nurse – by this icon marked are patients having entered (still not send 160 to the NFZ) a declaration to nurse
  • entered declaration to midwife – by this icon marked are patients having entered (still not send to the NFZ) a declaration to midwife
  • entered declaration from the scope of school medicine – by this icon marked are patients having entered (still not send to the NFZ) a declaration from the scope of school medicine
  • sent to the NFZ declaration to family doctor – by this icon marked are patients having sent to the NFZ (still not verified) a declaration to family doctor
  • sent to the NFZ declaration to nurse – by this icon marked are patients having sent to the NFZ (still not verified) a declaration to nurse
  • sent to the NFZ declaration to midwife – by this icon marked are patients having sent to the NFZ (still not verified) a declaration to midwife

  • necessity to update patient’s data - by this icon marked are patients with declaration in unit where declared staff no longer works
  • there are warnings about patient (person or treatment) - by this icon are marked patients who has entered warnings in the "Warnings" tab; Simply click on the icon to go automatically to the "Warnings" in the patient’s card in order to see the details
  • type of patient – marked by this information are patients classified to a particular type, e.g. VIP patients
  • patient registered by the ZnanyLekarz website - check / complete his data - this icon is used to mark patients added to the jHIS system when making an appointment via the ZnanyLekarz website, whose data should be checked / supplemented by registration on the day the patient arrives
  • patient registered by the ZnanyLekarz website - this icon is used to mark patients added to the jHIS system when making an appointment via the ZnanyLekarz website, whose data has been checked / supplemented by registration on the day the patient arrives
  • lack of declarations set - this icon is used for patients who do not have a complete set of health care declarations, i.e. women who do not have a declaration to a doctor or nurse or midwife and men who do not have a declaration to a doctor or nurse; the icon is displayed only when the system has been configured in such a way as to inform about deficiencies in the declarations
  • results of examinations from the entrusted budget / results of benefits / completed vaccinations waiting for discussion and/or settlement - this icon marks patients for whom the results of diagnostic examinations from the entrusted budget were received from the laboratory or for whom the results of benefits were registered, therefore visits should be planned for them in order to discuss and settle examinations/benefits, as well as patients who have received vaccinations which are waiting for settlement

When you hover the mouse over each of the icons there is a hint of what each icon means.

In addition, for patients having PESEL number (or PESEL number of tutor for neonates) above the form is displayed information about the patient's today insurance status in the eWUŚ - if the patient is insured, the information about it is green, but if the patient is not insured or have problems in communication with eWUŚ, relevant information is displayed in red. In addition to information about insurance, there may also be information about quarantine the patient. If eWUŚ, in addition to the insurance status, also returns additional information about the patient (e.g. that the patient is a refugee), then exclamation marks in a triangle are displayed next to the insurance status - when you hover the mouse cursor over such an icon, a balloon with additional information about the patient will be displayed. If the patient is insured on the basis of the provisions on coordination, then the patient's insurance status in eWUŚ is not verified, but only the appropriate message about the patient's insurance on the basis of the provisions on coordination is displayed in green.

Due to the entry into force of the regulations on e-services (e-prescription, e-referral), it has become necessary to register additional patient data. Therefore, for patients entered into the system prior to the entry into force of the said provisions, the system displays warnings about the need to complete missing data. If a patient insured under the provisions on coordination has an insurance document in the form of an EKUZ or a certificate, but the country does not have the indicated country, then the patient's card displays a message that the country must be completed in the insurance document in the "Insurance" tab. (If this country is not completed, it will not be possible to issue an eprescription and e-referral - when the doctor tries to issue an e-prescription / e-referral to a patient where the country of issuance of the insurance document is not set, the system will not allow to save and inform him about the need to complete the data.) If the patient's identity document is an ID card, passport or EKUZ card, but there is no document of the indicated country, then the patient's card displays a message that the country must be completed in the data of the identity document in the "Patient" tab (in the case of ID card it must be a Schengen country). (If this country is not completed, it will not be possible to issue an e-prescription and e-referral.) In the case of patients identified by a different document, when such a document is an EKUZ card, make sure to mark the box "EKUZ card" next to such document and fill in the box "Patient identifier (field no 6 in EKUZ card)", despite the lack of warnings from the system (the system, on the basis of the collected data, is unable to determine whether a given document is an EKUZ, hence no warning).

The form allows you to enter changes in patient’s data. After entering PESEL number, if PESEL number is invalid, frame around the field turns red, and if it is correct, then the fields date of birth, sex and type of patient will automatically be filled (if you have not filled them before entering PESEL number).

It is possible to enter a patient's photo: to add / change a photo, select a JPG file in the "Photo" field, to delete a photo (if it has already been entered), select the checkbox marked with a red "×" visible above the photo.

There is a button above the form that allows for issuing patient insurance statements, which are issued to the patient for signing in the case of an incorrect eWUŚ system response. After clicking the button "Patient insurance statements", the form for issuing a statement opens in a new window and under it there is the list of already issued statement. To issue a statement, complete the form and click the "Issue statement" button (if the patient's signature is to be registered for the statement, then when filling in the form, the "Request signature" box should be additionally checked). After issuing the statement, a link is displayed above the form that allows you one download and print it, it also appears on the list of statements under the form. If a signature has been requested for a given statement, then the signature is registered as described in the chapter "Account" in the sub-chapter "Signing documents". Next to each statement on the list of statements, under the form, there is a "View" button that allows you to download and print a given statement, and for statements for which a signature has not been requested, you can additionally see the "Request signature" button that allows you to generate such a request.

 

The statements have PDF format.

 

In turn under the form are following buttons:

  • Save - save changes and remain in the "Patient" tab of patient’s card
  • Save and exit - Save changes and return to the search engine of patients
  • Cancel – undo the changes and remain in the "Patient" tab of patient’s card
  • Back to patients search - back to search engine of patients without saving changes

Independently from used saving button before saving data to a database system verifies their correctness. In case of irregularities displays an error message indicating what should be corrected in the form. In case of such message data in the form should be corrected according to the message and retry to save. Correct saving ends with a message about the successful completion of the operation and displays patient’s data form (when to save the data is used "Save" button and form has not entered date of death of the patient), or displays a message about the successful completion of the operation and search engine of patients (when to save the data is used the "Save and exit" button or when to save data is used the "Save" button and form has entered the date of death of the patient). In the situation when the form has entered date of death of the patient, the patient will automatically become inactive and entering further modifications to his card is not possible.

Additionally, under the form, the "eWUŚ history" button is shown, presenting the history of checking for patient insurance in the eWUŚ system. After clicking this button, a new window opens with a list of checking dates and statuses.

 

In connection with the entry into force of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free flow of such data (RODO), buttons meeting the requirements of the Regulation are placed at the bottom of the tab. Button “Print personal data” allows to print for patient request his personal data that are processed within the system. Button “Export personal data” allows to export patient personal data into interoperable format (HL7 CDA) to transfer them to another entity. Button “Print consent (RODO)” allows to print consent to the processing of personal data to be signed by patient (after signing the consent, one has to state that fact by marking the field “Consent to the processing of personal data (RODO)” within the form and saving data).
